Description
This webinar is designed to improve overall hypertension management of patients and add specific emphasis on caring for Black patients with HTN. The SPRINT treatment will be discussed in addition to assessing for Social Determinants of Health (SDOH). The faculty will discuss criteria to identify patients with high blood pressure and how to engage them in follow up care. This will lead to improved rates of HTN control and improve patient outcomes.
