Plan a CME Activity

Educational activities must follow a prescribed planning process in order to be eligible for CME certification.

The following informational documents and planning forms have been developed by the OSU CCME in accordance with the ACCME® criteria requirements.

If you are looking to certify an educational activity as a Regularly Scheduled Series or an Enduring Material, please see the sections designated for those types of activities below.

CME Planning Process / Certification Forms for Live Activities

The following are documents used for the certification of all live activities with the exception of Regularly Scheduled Series, or RSS. If you are seeking certification for an RSS, please consult the Regularly Scheduled Series (RSS) Documents section below.

Application for Category 1 Credit Resources

In order to create an educational activity for AMA PRA Category 1 Credit(s)™, there is a documenation process.  The main document is not included in this section as each application is customized per the details of your specific educational activity.  Contact your CCME coordinator to obtain this form and to begin the necessary discussions. The following resources should aid you in the process as well as the onsite logistics of your event.

Joint Providership Forms

Joint providership CME activities are defined as those in which a faculty member approaches OSU CCME on behalf of a not-for-profit organization outside of the institution to certify a CME activity for AMA PRA Category 1 Credit(s). A faculty member, for these purposes, is a regular faculty member employed by either the University or the Wexner Medical Center in a teaching, research, or administrative capacity. The proposed program must be one for which at least one OSU faculty member serves in the primary role of initiating, planning, and overseeing the educational content and quality. In addition, the faculty member’s chair must recommend approval of the program. The program must also be approved by the dean or his/her decanal designee. The joint providership must be with an established scientific or professional organization. The activity or the organization must be consistent with the academic, research, and patient care mission of The Ohio State University Wexner Medical Center.

Conflict of Interest Disclosure Forms

Effective 2022, a universal conflict of interest (COI) form has been initiated by the ACCME(R). Therefore, the following form is to be used for all members involved in the execution of an activity. For example, planners, speakers, etc.
