Description
The Annual Oncogeriatric Symposium brings together academic experts and community clinicians to improve cancer care for older adults through shared learning. The program emphasizes community integration, combining the latest evidence with real-world perspectives to ensure care strategies are practical and adaptable across diverse settings. Through expert talks and case-based discussions led by community providers, participants collaborate on patient-centered approaches that align treatment with functional status, goals of care, and local resources, strengthening connections between academic and community practice.

Objectives
1. Evaluate the interplay between aging, comorbidities, social determinants of health, and cancer care delivery across community settings to better address the unique needs of older adults with cancer.
2. Analyze strategies for strengthening partnerships between oncology teams and community-based organizations to improve coordination of supportive services, including transportation, nutrition, caregiving support, and psychosocial resources.
3. Identify and disseminate best practices for integrating community services into cancer care.
