Description

Urinary tract infections are among the most common bacterial infections encountered in ambulatory practice. This case-based discussion will review the evaluation and management of a young woman presenting with classic symptoms of acute uncomplicated cystitis. Participants will explore the clinical diagnosis of uncomplicated UTI, differentiate uncomplicated from complicated infections, identify situations requiring additional testing or urine culture, and review current evidence-based treatment recommendations. The session will also highlight red flag findings suggestive of pyelonephritis or more serious infection requiring escalation of care.

Objectives

At the conclusion of this activity, participants will be able to:

  1. Recognize the clinical presentation of acute uncomplicated cystitis and identify patients who can be diagnosed and treated based on symptoms alone.
  2. Differentiate uncomplicated urinary tract infections from complicated infections and pyelonephritis using patient history, physical examination findings, and risk factors.
  3. Determine when diagnostic testing, urine culture, or additional evaluation is indicated, including recognition of red flags that warrant further investigation before initiating treatment.
  4. Apply current guideline-based treatment recommendations for uncomplicated cystitis, including appropriate antibiotic selection, duration of therapy, and patient counseling. 
  5. Identify patients who require escalation of care, specialty referral, or hospitalization based on signs of systemic illness, treatment failure, recurrent infection, or high-risk clinical features.
