Objectives
As a result of this educational activity, webcast participants will be able to: DESCRIBE the need for successful health care transitions for youth with special health care needs (YSHCN); SUMMARIZE the goals of transitional health care for YSHCN; DISCUSS the challenges and barriers to successful transitions for YSHCN; SUGGEST what physicians can do to improve the transition process for YSHCN; and UNDERSTAND available tools to help patients prepare for and successfully transition from pediatric to adult care.Webcasts Files
